Do I need to be
referred by a physician?
|
Yes. In
Missouri PT and OT are only available on orders from a licensed
physician. You need a referral from an MD, DO, Dentist,
Podiatrist, or Chiropractor. |

What should I
bring to my first visit? |
You will be
asked to fill out a patient information form which includes
information about your insurance, and if your condition was
accident related, about the responsible party and any attorneys
involved in your case. Please bring your insurance card and all
address and telephone information for attorneys, employers, etc
You should also
bring a list of all medications you are currently taking including
the dosage, and why you are taking the prescription. |

What should I
expect at my first appointment? |
Your therapist
will perform a thorough evaluation of the affected area and
develop a treatment plan which will include both your care in the
clinic and a personalized home exercise program. |

What should I
wear? |
You should bring
or wear comfortable clothing. Those experiencing upper extremity
problems will find a tank top most appropriate. For lower extremity
problems shorts will be best. We have gowns available for those
with back or neck conditions. |

How long will my
appointments be? |
You should
expect your first appointment to be 45 – 60 minutes with
subsequent visits being 30 – 60 minutes. |

Does my
insurance cover physical and occupational therapy? |
Heritage
Physical Therapy is pleased to participate as a provider for the
vast majority of plans available in the
St. Louis / St. Charles and
surrounding areas. You should verify your benefits with your
insurance plan directly. You can also see many of the more common
plans in which we participate by visiting our
insurance plans. |

Will you verify
my benefits? |
We will contact
your insurance company during your first visit and can tell you what
your insurance company has quoted as your benefits. We are not,
however, responsible for differences between what they tell us and
how they actually pay your claims. We urge you to contact your
insurance company directly for benefit information. |

Will you bill my
insurance company? |
As a courtesy to
our patients we do file claims with the insurance company, provided
we are given appropriate information regarding your insurance.

Does Heritage
Physical Therapy accept assignment on Medicare? |
Yes. We are a
Medicare Participating Provider. |
|
When must I pay
my co-pays and deductible? |
We encourage
patients to pay their co-pay at the time of service. In instances
where there are deductibles or co-pays remaining we will send a
bill as the insurance claims are paid. |
